Billing Services/Payment options – 

Transformation Therapeutics takes the following insurance:

• Medicare
• Medicaid Colorado
• Pinnacol Workman’s Compensation
• Workman’s compensation
• United Health Care
• UMR

We are diligently working on becoming in-network with insurance companies. Transformation Therapeutics will accept cash payments and will provide an itemized statement that can be filed with an insurance company. Our out-of-pocket payment costs $50.00 for every 15-minute evaluation time and $33.00 for every 15 minutes of treatment. On the initial visits, most sessions will consist of 30 minutes of evaluation and 30 minutes of treatment totaling $166.00. The time wanted on subsequent sessions can be determined by the patient for the amount wanted for out-of-pocket costs.
